REMINDER TO ALL ATHLETES AND PARENTS:
As of May 31, 2024, the NJSIAA and the DJDOE have approved a new physical form and process in order for students to participate in extracurricular activities. Upon completion of a meeting with our school district physician, Dr. Thomas Bottiglieri, today, June 17, 2024, to review these new procedures, we will be instituting the following:

As of July 1, 2024, ALL physicals must be completed on the new forms attached, you may begin bringing them in immediately. The old forms will no longer be accepted after July 1, 2024.

The packet is available in the Main Office and will also be available at the Bear Cave during the month of July while the High School is closed.

Please bring the packet to your personal doctor to complete. Your physician should complete all of the pages including the last page.

The only page that needs to be turned in to the school or the trainer is the final sheet, marked on the top in Red “Please Hand In Only This Page To The Main Office or To The Trainer”, all other pages should remain with your physician.

Please do not turn in any other pages of the physical packet.

If your child requires an AAP, Asthma Action Plan, or any other EAP, Emergency Action Plan, as per their physician, please make sure a copy of that is attached to that final page upon submission.

Once that page is turned in, our trainer will bring it to our school physician and you will receive official CLEARANCE notification through our Rschool registration. Please be reminded ALL STUDENTS MUST REGISTER PRIOR TO HANDING IN A PHYSICAL.
We want to thank you for your understanding with this change in procedure. We are well aware that we spoke about the old physical form during the Fall Sports Parent Information Night, unfortunately, we were not notified until after that meeting that the forms would be changing.
PHYSICALS ARE GOOD FOR 365 DAYS IF THEY WERE APPROVED BY THE SCHOOL DOCTOR.
