Hello Everyone, my name is Ms. W and I am the Athletic Trainer of the Hawthorne Bears! I am the healthcare professional in charge of all student athletes. I have the ability to help prevent, educate, evaluate, and rehabilitate injuries for the athletes at Hawthorne as well as provide immediate care and coordinate care when needed. I have both my Bachelors and Masters degrees from William Paterson University and am currently in my 4th year as a Board Certified and licensed athletic trainer. I am super grateful to be a part of the Bear family!!
Policies & Procedures
Hawthorne High School Athletic Training Policies and Procedures
Physicals:

All athletes must have a physical from within the last 365 days on file before participating in sports.

Physicals must be completed on the NJ state forms. Universal forms will not be accepted.

If your son/daughter uses an inhaler and/or Epi-pen, their physician must fill out an asthma action plan and/or Epi-pen use form. This needs to be completed every year.

Rschool Registration:

Every athlete must be registered on Rschools for each sport.

When creating an account, please use your personal email and use a password you will remember. One account needs to be used for all 4 years and can be used for multiple children.

Please fill out all forms completely EXCEPT for Step 4. Physical forms DO NOT need to be uploaded. Please skip step 4 entirely.

You will receive an email from Rschool once your child is fully cleared for their sport. If you do not receive an email, this means something is missing and please reach out to the athletic trainer to figure out what.

Concussions:

All athletes are required to take a baseline concussion assessment prior to the start of the season.

Any athlete who sustains a concussion will be excluded from participation until a physician who specializes in concussions evaluates the athlete.

Once the athlete obtains clearance, the mandatory 5-day progression back to sports will begin. The athlete WILL NOT be permitted to participate immediately.

Injury Procedures

All injuries MUST be reported to the Athletic Trainer immediately. This is to ensure that proper care is given to the athlete and proper instructions for home care are provided.

If the athletic trainer recommends further medical evaluation, a sports medicine or orthopedic physician should be consulted. This can be coordinated through the athletic trainer should you choose to use the school doctor.

If at any time during the season your son/daughter goes to see a doctor for any reason, please note that a note from the doctor allowing him/her to return to sports should be submitted to the Athletic Trainer before the student-athlete resumes activity.

A doctor’s note must be submitted to the Athletic Trainer when the athlete is ready to return to activity. We will not accept any notes from the emergency room.
